Clearer Care
Clearer Care is a Surrey-wide health literacy pilot programme designed to help organisations make health information, services and communication clearer, more accessible and more inclusive for local communities.
VCSE (Voluntary, Community, and Social Enterprise) organisations based in Surrey, working to improve public health outcomes and statutory bodies (e.g. NHS Trusts, local authorities) are eligible to apply.
Through expert support and grants of up to £2,500, the programme will fund pilot projects that improve how people understand and use health and care services.
Grant funding is only available to VCSE organisations working in Surrey.
What the programme will support
Many people struggle to understand health information, navigate services, or know when and where to seek support. This can lead to missed appointments, poorer health outcomes and widening health inequalities. Clearer Care aims to tackle these challenges by supporting practical, community-based projects that help people better understand and use health and care services.
The programme will strengthen people’s ability to access, understand, evaluate and use health information, empowering them to make informed decisions about their health and wellbeing.
Clearer Care will fund and support pilot projects that improve health communication and understanding. Projects could include, for example:
Helping people understand how to manage long-term health conditions, such as diabetes
Making it easier for parents and carers to understand how and where to access childhood immunisations
Improving clarity about available services for people living with long-term conditions
Developing clearer, culturally appropriate or accessible health information for underserved communities
Projects should be community-focused and designed to address barriers faced by vulnerable or marginalised groups. We encourage partnerships between VCSE organisations & statutory bodies, to deliver health literacy pilot projects.
Who Can Apply
Clearer Care is open to organisations working in Surrey, including:
Voluntary, Community and Social Enterprise (VCSE) organisations
NHS organisations
Local authorities and public health teams
Grant funding of up to £2,500 is available to VCSE organisations only, either applying independently or in partnership with a statutory organisation.
Projects should focus on improving health literacy, access to services, or understanding of health information for Surrey communities, particularly those facing inequalities or barriers to access.
Support Available
Organisations selected for the programme will receive:
For all participating organisations:
Expert advice and guidance from a health literacy specialist
Practical support to design, deliver and evaluate pilot projects
For VCSE organisations ONLY:
Small grants of up to £2,500 to deliver pilot projects (e.g. staffing, materials, engagement activities)
VCSE organisations can apply on their own or in partnership with an NHS or local authority ‘buddy’ organisation. If you have an idea but no statutory partner yet, support will be available to help make those connections.

Funding decisions: June 2026
Pilot delivery period: July–December 2026
