Preventative Power – showcasing ways to collaborate and fund work on wider health determinants

Community Foundation for Surrey is celebrating the publication of an important document – Preventative Power: Community Foundations Partnering with Health.

 

This really important report showcases examples from Community Foundations all over the UK who are working in partnership with their local NHS to fund and support local communities to develop and implement work that will help communities to address some of the key underlying issues that too often can lead to health crisis.

Wider determinants of health, also known as social determinants of health, are the non-clinical factors that significantly influence a person's health and well-being. These include social, economic, and environmental conditions that shape the conditions in which people are born, live, work, and age.  We know that these conditions have a huge influence on the wellbeing of a person – and we know that very often local community groups, charities and similar organisations have the best knowledge and expertise to develop programmes to address these issues. 

 

Partnerships between the NHS and Community Foundations have been instrumental in developing really impactful programmes that address key issues: 

  • Moving care from acute hospitals into the community if that results in better outcomes. 

  • Focussing on prevention of health crises by supporting earlier intervention to develop good habits and address key needs. 

 

The case studies highlighted cover a very wide range of programmes that address issues such as mental health and wellbeing, addressing loneliness, food poverty, access to debt advice or affordable accommodation. All these are going to have a profound affect on someone’s health – and the more we can do to address them, the more we can take pressure off acute healthcare such as A&E, which will be vital if we are to enable our wonderful NHS to be able to cope with the health needs of our population.
